Enrollment Requirements
New York State Health Law requires all children that attend daycare and/or
preschool be up to date with immunizations.
The following forms will be provided and must be completed before your
child attends:
DAYCARE PRESCHOOL
Provider-Parent/Guardian
Child Care/Preschool Agreement X X
CACFP Enrollment Form X
Medical Form X
Medical Report (completed by physician) X X
Registration Card X X
DSS Contract and/or Casworker's X
name & number, if receiving child
care assistance
